Vanderbilt-Ingram Cancer Center has initiated tumor mutation testing for a limited number of patients with metastatic colorectal cancer.

This pilot project for colorectal cancer is part of VICC?s Personalized Cancer Medicine Initiative (PCMI), a program to identify genetic mutations in a patient?s tumor that may be useful in matching the appropriate therapy with each patient. VICC patients with lung cancer, breast cancer and melanoma already have their tumors tested for specific mutations, and now some colorectal cancer patients will be added as part of a pilot program.

?We?re testing a panel of eight genes, including mutations in the KRAS and BRAF genes,? said Emily Chan, M.D., Ph.D., assistant professor of Medicine and clinical director of GI Oncology. ?What we?re looking at are genes that are known to be mutated in some colorectal cancers. Companies are already developing drugs that block the molecular pathway that gets activated in some of these gene mutations. Other genes don?t have drugs available at the moment, but that doesn?t mean they won?t down the road.?

Chan points to the drug vemurafenib, which has shown strong results in many melanoma patients with the BRAF mutation. About 10 percent of colorectal cancer patients also have the BRAF mutation in their tumor.

?We have participated in a trial looking at a small group of colorectal cancer patients and we did not see the same impressive responses that were seen in melanoma, which lets us know that colorectal cancer is a different disease than melanoma, and vemurafenib by itself doesn?t get the same impact. So it may need to be given in combination with something else. Knowing more about the different mutations within the tumor may help guide us to what may be the best treatment combination for a particular patient,? Chan said.

Some of the mutated genes may serve as prognostic markers to predict how patients will respond to a specific therapy. The results may also help VICC physicians match patients to clinical research trials that target certain molecular pathways in the tumor.

The gene tests are conducted on tissue samples from the patient?s tumor. To participate, patients must have enough tissue available for testing and they must sign a consent form to allow for the mutation testing. About 200 patients will be tested during this initial colorectal cancer program and the results will be evaluated for future projects.

?I have patients who are very interested in getting this testing done, even if they?re on some current therapy because they?re trying to plan for the future. The other thing I hear from patients is even if it doesn?t help them it may help someone in the future, so there?s an altruistic aspect to their willingness to help, as well,? said Chan

San Bernardino Bankruptcy: City Declares Fiscal Emergency

SAN BERNARDINO, Calif. -- San Bernardino declared a fiscal emergency Wednesday night, allowing the city to avoid a lengthy mediation process and head straight to federal bankruptcy court.

The declaration comes after the city announced last week that it would seek Chapter 9 protection, making it the third California city in recent weeks to make the rare move.

The City Council voted 5-2 to declare the emergency and file for bankruptcy protection amid a dire cash crunch that has officials worried San Bernardino can't meet payroll in August.

The vote was followed by another authorizing the city attorney to file for bankruptcy, but it was not clear when the planned filing would come.

Councilmen Chas Kelley and John Valdivia dissented on both votes.

Councilman Fred Shorett, who voted against bankruptcy last week, reversed his position Wednesday night and approved both moves.

"The horse is out of the barn ? the whole world knows we're insolvent," Shorett said, according to the San Bernardino Sun. "I will be supporting going forward with Chapter 9 and fiscal emergency."

The vote could make the city of 210,000 people the third in California to seek bankruptcy protection since last month, following Stockton and Mammoth Lakes.

The city is facing a $45.8 million budget shortfall this year.

Last week's announcement of the bankruptcy plan has further stressed San Bernardino's finances by prompting a dozen employees to put in for retirement with hopes of cashing out accrued vacation and sick time, and it has spurred vendors to demand cash instead of credit, said Gwendolyn Waters, a spokeswoman for the city manager's office.

The debate over bankruptcy in San Bernardino has also raised questions about the city's financial management. Last week, City Attorney James Penman told the public that 13 of the last 16 budgets presented to the city council had been falsified, masking the city's deficit. The finance director, who is new to the job, said officials had borrowed cash from restricted funds to cover payments, and eventually ran out of money to pay the funds back.

Officials say the housing crisis ? which walloped property and sales tax revenues ? and the loss of state redevelopment funds took a toll on the city's budget. San Bernardino is located about 60 miles east of Los Angeles.

ScienceDaily (July 20, 2012) ? Severe influenza doubles the odds that a person will develop Parkinson's disease later in life, according to University of British Columbia researchers.

However, the opposite is true for people who contracted a typical case of red measles as children -- they are 35 per cent less likely to develop Parkinson's, a nervous system disorder marked by slowness of movement, shaking, stiffness, and in the later stages, loss of balance.

The findings by researchers at UBC's School of Population and Public Health and the Pacific Parkinson's Research Centre, published online this month in the journal Movement Disorders, are based on interviews with 403 Parkinson's patients and 405 healthy people in British Columbia, Canada.

Lead author Anne Harris also examined whether occupational exposure to vibrations -- such as operating construction equipment -- had any effect on the risk of Parkinson's. In another study, published online this month by the American Journal of Epidemiology, she and her collaborators reported that occupational exposure actually decreased the risk of developing the disease by 33 percent, compared to people whose jobs involved no exposure.

Meanwhile, Harris found that those exposed to high-intensity vibrations -- for example, by driving snowmobiles, military tanks or high-speed boats -- had a consistently higher risk of developing Parkinson's than people whose jobs involved lower-intensity vibrations (for example, operating road vehicles). The elevated risk fell short of the statistical significance typically used to establish a correlation, but was strong and consistent enough to suggest an avenue for further study, Harris says.

"There are no cures or prevention programs for Parkinson's, in part because we still don't understand what triggers it in some people and not others," says Harris, who conducted the research while earning her doctorate at UBC. "This kind of painstaking epidemiological detective work is crucial in identifying the mechanisms that might be at work, allowing the development of effective prevention strategies."

The disease typically strikes people over age 50. SANTA CLARA, Calif. (AP) - Marvin Chavez scored against his former team and Alan Gordon added his eighth goal of the season to lead the San Jose Earthquakes to a 2-1 victory over FC Dallas on Wednesday.

San Jose had to withstand an 89th-minute goal from Hernan Pertuz and a missed penalty kick in the 94th minute by Jair Benitez.

The Earthquakes (13-4-4) widened their lead in the Supporters' Shield race to seven points clear of Real Salt Lake and Sporting Kansas City. San Jose is unbeaten at home in Buck Shaw Stadium since Aug. 13, an 8-0-3 run.

FC Dallas (4-10-7) ended a 13-match winless streak Saturday, but couldn't continue its momentum, despite the debut of newly acquired designated player Julian de Guzman, traded last week by Toronto FC.

Chavez, who was dealt to San Jose from Dallas in the offseason for allocation money, struck from the edge of the penalty box in the 12th minute, ripping a well-placed shot to the far post off a skidding cross by Shea Salinas.

Gordon doubled San Jose's advantage with a header in the 45th minute, also beating Dallas goalkeeper Kevin Hartman to the back post. Benitez started the play with a poor pass that led to a turnover deep in Dallas' third of the field. San Jose made three passes through the midfield before the ball reached Sam Cronin, whose looping pass from 35 yards found the loosely marked Gordon.

Chris Wondolowski, MLS' leading scorer, appeared to strike for his 18th goal of the season on a 61st-minute penalty brought about by Brek Shea's clear hand ball in Dallas' box. But San Jose was called for encroachment on Wondolowski's successful first attempt, and Hartman dove to his left to save the redo.

Pertuz got Dallas within a goal on a flying header, but Benitez gave away a chance to steal a road point after Zach Loyd was bundled over by San Jose midfielder Simon Dawkins in second-half injury time. With Brek Shea, Dallas' usual penalty-kick taker, having been subbed off, Benitez stepped up to the spot and flashed his attempt just above the crossbar.
